[eMINTS] Parent survey

This is my first year as an eMINTS teacher.  I'm interested in sending 
home a parent survey at the end of the year, surveying how my parents feel 
their child has grown educationally and technologically over the past 
school year by being an eMINTS classroom. 

Does anyone out there do something similar or any kind of parent survey. 
If possible, could you attach the file to the email???
