[eMINTS] Special Ed tool to help create needed program

The Speech and Language Pathologist who works for our school is looking
for a software program that will allow him to create a tool that he
needs for his record keeping. He would appreciate any information or
software suggestions you might have. Below is a paragraph written by him
explaining what he needs the tool to be able to do for him. Thanks for
any help you can provide him.

Here is a description of what I need this program to do.  Thanks for
your help,

  I am searching for a user-friendly, multifaceted program that allows
me to input student data (i.e. name, grade, birth date). This data needs
to then be automatically inserted into each subsequent form that is
generated. After entering the demographic info, I would like the program
to present me with a check box list that allows me to select specific
concerns with a child. For example, I would like to be able to select
articulation, language, and fine motor concerns. Based on the selected
concerns, I want to be presented with a list of commonly used evaluation
tools (tests). I would like to select the tests that I will administer
using a check box. Then I want the program to present me with a template
for each of the selected tests that I can input objective results from
the test. From there, I would like that information to be inserted into
a narrative type form that is IEP friendly. I would also like the
objective data to be automatically inserted into a section on the
 "Present level" form for an IEP. I would then like to be prompted
for a recommendation regarding services and minutes for the IEP. After
making a recommendation, I would like to be shown a bank of goals that
corresponds to the child's identified deficits. For example, if the
child demonstrated deficits in the area of language I would like to see
a list of commonly utilized language goals. Once I select appropriate
goals from the goal bank, I need the program to plug those goals onto an
IEP goal form.
